1. How are you using your inhaler? How often do you use rescue inhaler?
2. How long have you had your cough? (>3 months)
3. Any sputum (mixture of saliva and mucus)?
4. Are you a smoker? (are you willing to quit?)
5. Any SOB? (how frequent? [Usually on exertion]), Do you have any chest heaviness?
