1. Out-patient, oral
    1. Macrolide or doxycycline
    2. Respiratory fluoroquinolone  or combination: amoxicillin or amoxicillin/clav (or cefpodoxime or cefuroxime) WITH macrolide (or doxycycline) – see IDSA guideline
  2. In-patient (non-ICU), IV
    1. Ceftriaxone or cefotaxime or amp or ertapenem WITH azithromycin or doxycycline – see IDSA guideline
    2. Respiratory fluoroquinolone
  3. In-patient (ICU), IV
    1. Complicated; depends on risks for Pseudomonas, and/or MRSA – see IDSA guideline
    2. Beta-lactam+macrolide or respiratory FQ
    3. For psuedomonas: piperacillin/tazobactam (or cefepime, imipenem, meropenem)+ciprofloxacin or levofloxacin

CAP pts should be treated for at least 5 days
